A Case of Back Toric Rigid Gas Permeable (RGP) Contact Lens for Correction of Irregular Astigmatism of Severe Keratoconus

Abstract
Purpose: To report a case of correcting irregular astigmatism in severe keratoconus with back toric rigid gas permeable (RGP) contact lens.

Case summary: A 43-year-old man visited our clinic for correction of decreased visual acuity on the both eyes. The refractive status of both eyes was over mean 5 diopters. The best corrected visual acuity (BCVA) of right was 20/25 and BCVA in left eye was 20/100. The corneal astigmatism was 1.75 diopters in right eye and 6.5 diopters in the left eye. By topography of both eyes, the right eye has minimal degree of keratoconus and the left eye was more advanced stage of keratoconus. He could not wear the RGP contact lens with multi-curved or high eccentricity due to corneal irregular astigmatism. Using back toric RGP contact lens, the right eye¡¯s BCVA was improved 20/25 and left eye¡¯s BCVA was improved 20/40. There were no significant complications or abnormal finding in the corneal surface.

Conclusions: Back toric RGP contact lens appears to be effective in improving BCVA and reducing corneal irregular astigmatism of keratoconus when multi-curved or high eccentricity RGP contact lens failed.
